Most likely yes!: It depends on the reason you have a cast on and the initial injury and the time from when the cast was placed as well as the exact surgery (i assume it's not for a hemodialysis fistua creation). As for IV access, your other extremities can be used. Best is to check with the facility that initially applied the cast to your hand to be sure. Overall, having a cast on does not interfere with surgery.

Splitting a cast: Using a cast saw is a way to make sure that swelling if present will not allow the cast to constrict too much. If that is a concern you may ask your treating hand or orthopedic surgeon if that can occur if you have anesthesia.

Yes: It's possible to have other surgeries with a cast on. The IV can be placed on the other hand or leg. The nurses will make sure the cast is well supported during surgery.
