Start with a: Medical degree & internship. Many have also completed residencies in a specialty. It isn't uncommon to have a masters of business administration in health care (mba). I have known some executive physicians with masters in public health and have known some who did not have administrative degrees.

No legal requirement: Some physicians feel more comfortable in an executive role and some get burned out with treating patients. There is no legal definition of the requirements for an executive physician. Many who seek this track will get an MBA or legal degree to help their chances.
