See urologist: A plastic surgeon has no role in this sort of surgery or management. If you have seen a urologist already then they would have done an ultrasound or a scan to make a diagnosis. If you need a biopsy to get a diagnosis it would be performed by them. Orchiectomy by definition is removal of testicle and that is usually considered for a diagnosis of malignancy or for a gender reassignment surgery.
