Both: Both injuries can be debilitating for a pitcher. One is not necessary worse than the other, and there are a variety of things that can occur at each joint.

Either.: Either could be worse, depending on what structures are damaged. An ulnar ligament injury to the elbow is very disabling to a pitcher. A labral tear in the shoulder could be very disabling to a pitcher.
