Many reasons: There could be many reasons for your symptoms ranging from traumatic or non-traumatic causes. You don't need to have an injury to experience the symptoms you have described. Common causes include early arthritis/wear/tear, ligament injury or meniscus tear. If you are not satisfied with your recovery and if symptoms are unbearable or worsen then see an orthopedic surgeon for clarity in diagnosis.

Arthritis: Whatever you did 4yrs ago undoubtedly damaged the knee. Running is pretty hard on knees. Arthritis, inflmation of joints, results from cummulative damage. Stregthening muscles with weights, making sure joint alignment is optimal, optimizing running technique and icing will slow/help the process. Lots of knees get replaced due to wear and tear.
