Complex: Chronic immune mediated inflammation of the glomeruli (filtering units), does them in over time and causes loss of ability to clear the daily toxin and water load.

Scar formation: Chronic glomerulonephritis (regardless of the cause) is chronic inflammation which will lead to scar formation (aka fibrosis) in the kidneys. Fibrosis is a reperative or reactive process which turns functional elemants of the kidneys into nonfunctional fibrous tissue and hence kidney failure.
