Depends on age and: type of bilirubin. High bilirubin is most dangerous for newborns and at a level of 20 mg/dL of higher they can get irreversible neurologic damage. In adults it depends more on the cause of elevated bilirubin than bilirubin level per se. http://www.nlm.nih.gov/medlineplus/ency/article/007309.htm
