Shouldn't: If the trach is in, shouldn't happen unless something is wrong with trach or there is another airway issue going on below the trach. Discuss with your doctor.

Possible: Stridor with a tracheostomy tube in place implies obstruction, either in the tube or in the trachea below. Another cause could be the tube not being in the right place. Otherwise, one should not have stridor with a tracheostomy tube.

Complications: Complications related to chronic tracheostomy such as granulation tissue, stenosis , or web will cause stridor following extubation. Airway should be evaluated by imaging or endoscopy before extubating patient to avoid subsequent ventilation problemsl.
