Any pain: Long standing ingrown toenails most likely is due to poorly fitted shoes narrow at the toe box.If you are able to put up with needless pain nothing will happen, if you have infection , pus coming out of edge of nail , then will have all the complication of septic focus in the body, if you have implanted devices, defective heart valves, vascular grafts they get infected , could be lethal.

Infectipn: If an ingrwon toenail is ignored an infection may develop usually requiring oral antibiottics. In rare instances if an ingrown toenail is ignored and infection occurs for a prolong period of time the bone may be infected leading to extensive surgery and prolong antibiotics.
