24-48 hours: It needs surgical intervention - meanwhile apply warm, wet compresses for 10 minutes 3-4 x a day. The antibiotic may take 24-48 hours to work. If it's not helping by then, it may not be covering the germ and would need to be changed. (you need staph coverage; maybe even mrsa).
