Possible: Depending on the cause of the pain, the pain can either be localized or can be referred to other parts of the body. Although, naturally upper back pain would be referring itself to the upper extremities or head, lower extremity pain due to the pain radiating down the spine is still possible.

No: Upper back pain generally refers to the neck or upper arms. Scoliosis can cause pain above and below the curve leading to upper back and buttock pain at the same time or alternating. See a physiatrist or chiropractor for help if the problem limits you. Change in your activities may be helpful.
