Conservative care: The majority of patients with back pain will respond to conservative treatment, including physical therapy and nsaids (i.e. Ibuprofen, aleve). A prescription for a steroid dose pack, as well as im injections (i.e. Toradol, and/or steroid) can be very effective initial treatment for acute pain. Mri can help to define the spinal pathology, where appropriate.

Things to consider: Sometimes sciatica involving the L5 and s1 nerves from the lumbar spine will refer pain to the gluteal muscles, tensor fascia lata/it band muscle. This can be diagnosed with a) MRI lumbar spine, b) lumbar epidural steroid injection for diagnostic and therapeutic reasons.

Workup: Sounds like you need a good workup for your pain by either a sport's medicine physician, pmr, osteopath, or pain management specialist to work you up and get you appropriate treatment. Unfortunately many pcps are not well trained in musculoskeletal conditions and just put patients on pain meds without finding a cause for the pain.
