Herniated disc: What is being described are two areas of disc protrusion but in one disc space meaning that you have a single disc space which has two areas of herniation, one mild and one moderate.

One disc level: Going just on the description, it appears that there is a right sided disc protrusion/bulge with another one that is more centrally located on top of the 1st one. So..Two disc herniations at one level.
