Wrong antibiotic: Most of the time. In fact, the majority of ingrown toenail infections do not need an oral antibiotic if the offending portion of the nail is removed.

Surgery most likely: Toe nail infection with ingrown toe nail usually heal faster with surgery. Removing the part of the nail and letting the body heal itself works well. Antibiotics play a supportive role.
