Is a soft big bump signs of a head injury?

No: A bump is a sign of a good hard knock but doesnt indicate presence of head injury or tbi. A head injury usually includes headache, confusion or altered conciousness if in doubt take this one to the er.

Head injury: External signs of head trauma suggest the possibility of brain injury via the same mechanism. However, the skull separates the scalp from the brain, and what's been injured on one side of the skull doesn't necessarily predict the other.
