Many possibilities: Lyme is a complex illness. Many with lyme have co-infections like babesia & bartonella, as well as chronic sinusitis, intestinal parasites & candidiasis. Antibiotic treatment can increase symptoms by creating die-off (toxicity from killed bacteria) and imbalances of gastrointestinal flora, leading to food allergies. Like all illness, stress can also aggravate symptoms and having lyme is stressful!
