Mental Health: If all is normal on exam and with testing we must face that this may be a problem being caused by your mind. I would advise a full and thorough psychiatric evaluation as part of the work up for your problem. Something may be going on in your psyche.

Syncope: You don't give enough details to speculate with any degree of accuracy. If you have had a full workup of medical tests, you may have hyperventilation or panic attacks. If you haven't had a full evaluation, then something more physical could still be a consideration.
