Fat deposits: As the condition of the liver deteriorates with advancing alcoholism, its ability to deal with fats in the blood (lipids) decreases. The extra fat is deposited beneath the skin in areas with a lot of sebaceous glands. Since the skin in the nose is both thin across the cartilage and also has a lot of glands, the buildup there actually changes the shape of the nose.
