Not usually: A hydrocele (abnormal fluid collection in the scrotum) is usually not painful but, if large enough, can cause aching, pressure, and discomfort, especially when wearing tight clothing or when physically active.

May be...: A hydrocele inside the cord (meaning in the inguinal canal) is more likely to cause pressure to surrounding tissue/nerve due to closeness to the pants and accidental pressure, unlike that inside scrotum where gives it more leeway to dissipate similar pressure. So, there - in the cord, it may be more painful or tender at times.
