Maybe not go ?: If a person with known afib has a heart rate is controlled (i.e. Less than 100 bpm), they are on a stroke reduction medication such as Pradaxa (or xarelto, Eliquis or coumadin), and if otherwise without significant symptoms, some people would call their doctor office the next morning that they are back in afib. Others with symptoms or any concerns would go in to the er for evaluation. Also is ok.

No clear cutoff : If symptoms are minimal or nonexistent then the er may not even be necessary. Depending on clinical history, a person may spontaneously revert to sinus rhythm without intervention. If this does not occur spontaneously then cardioversion may be necessary, however since you are on therapeutic anticoagulation therapy with pradaxa, there is no need to have this done within the 48 hour window.
