I took concerta 18 mg xl then 36 mg xl but none worked. should i change the medication ( in kuwait we only have concerta xl and xr and ritalin (methylphenidate) ).

Concerta: You need to discuss this with your medical provider but you may not be on a high enough dose yet. Some people may respond at 36mg but many will not.

Careful: As you may be aware, Concerta is an extended release form of ritalin. You may discuss with your doctor the timing of your meds and doses can go to 54mg and even 72mg in some but need to monitor for effectiveness and side effects. Some people can benefit from also adding wellbutrin (bupropion) (but again, consult your doctor). And of course, careful of addiction, misuse and dependence.

Concerta: Upgrading by 18 mg. At a time, or supplementing with clonidine, Kapvay or tenex (guanfacine). Review with your doctor. Salam.

ATTENTION: Starting dose of concerta: 18 mg, usually in younger kids; then titrated upward: 27, 36, 54, 72 mg. Most common dose in school children 36 mg with symptom control. As you are 20 yr, this dose is less for you, not working. I will go up to 54 mg and wait to see whether you need 72. Other option: if 54 not good enough, i'll add Guanfacine 1-2 mg. Other many combinations there, talk to your doc.
