Lab Error: Only lab error. If you want to make sure no errors are made, you repeat the test a 2nd time for confirmation or to refute.

Not much.: Telling which of two identical twins is the father is a challenge, of course; also, "mosaicism" - having 2 different genetic populations of cells in one body - is possible; a sperm could come from one population, and the tested cell from another. And there is also mishandling a specimen.
