Not gaining weight: I cannot tell you about your options without knowing what has already been done. If he has hypothyroidism and poor growth profile, i would think it is a hormonal related problem if his thyroid has not been well regulated. Not sure how GI would assess growth hormone, when endocrinology seems to be the firs line of reference for that.
