De Quervain's: Thumb pain with range of motion in fortyish female suggests de Quervain's tenosynovitis of thumb extensor tendon. In this condition exquisite pain can be aroused by someone suddenly forcing down the thumb held in the "hitchhiker's" position. This is only one possible explanation and, if correct, a single corticosteroid injection into tendon sheath can perform wonders. Causation? Who knows?
