If an mri report states that the radiologist recommends a biopsy can a pcp not follow through because they disagree?

Yes: Primary care doctors are at liberty to refer as they see fit. It is the "practice" of medicine. Radiologist make recommendations from what they see so that other doctors know if there is further concern they can order more tests. Depending on the clinical picture, it may not be worth the risk of a biopsy for example.

May be for a reason: Pcp may have reasons , like risk out weigh benefits , speak to your pcp again , then if you are not satisfied , take a second opinion , your pcp may even help you to get one.

Yes but why...: Although there is an occasional misunderstanding between the radiologist and pcp, most of the time the pcp has clinical information about the patient (that the radiologist does not) and is in the best position to make the most useful follow up recommendation. However, i think it would be a good idea for the pcp to explain the reason. If needed, a call to the radiologist might help.
