More history: Unfortunately, given the complexity of your problem, i would suggest that you see a doc who would be able to obtain more history data as well as do a thorough physical exam and hopefully will be able to further direct you in your work up and treatment.

Likely muscular: Pain distribution sounds muscular - except for the abdominal pain. Ultrasound of the abdomen (looking for gall stones or liver cysts for instance) may be helpful. Does the pain get worse after eating? Parasitic infections can cause this combination of symptoms (not uncommon where you live). See a doctor for a detailed history and physical which hopefully will lead to the diagnosis.
