Are there any effective ways to wean off smoking heroin?

Unlikely: Since smokable heroin use is so immediately reinforcing, and the dose of smoked heroin would be difficult to control. Better to switch to a longer acting opioid, or other medications to block withdrawal. I recommend professional treatment with medication to help detox from heroin. Methadone or Buprenorphine are often used for detox; Clonidine and tramadol are other options.
