Sounds like asthma: 4 months is too long you want to see your lung doc to examine you. You may need a cxr to make sure nothing else is going on. Hopefully this is not tb.

Infection pneumonia: You have a 4 month cough. It hurts to cough and makes a musical sound after sneezing. You are in nigeria. This is most likely infection and could be pneumonia. At 4 months start with seeing a doctor for exam and x-ray. You may need antibiotics and blood test. The musical sound is from delayed or trapped air through mucus. Be well and please take care.
