Infection or injury?: Did you injure the area? A bad bruise or fracture can cause this pulsation. Neuroma can also feel like this too. They are rare between the 1st and 2nd toes but can happen. Check circulation too, that may be an issue. Are you a smoker? Smoking can add to this issue. See a podiatrist if it doesn't stop in 3-5 days. Check your shoes too if they don't fit properly they can cause problems.

Morton's neuroma: I assume we are talking about the 2nd and 3rd toes? If the pulsation is without question, you likely have a neuroma. This is very common and most of experience such symptoms by the age of 55. Pain on the ball of foot with potential 'fullness or 'lump' impression, is without question a neuroma. Google this pathology.
