Dr. Jay Ryan answered

Pain: Inability to weight bear swelling pain bruising not all symptoms need to be present. Please get an xray. Even if you think it might be broken, you should have an xray to make sure. It can be a lifelong disability if you choose to ignore it. Even severe ankle sprains can take over a year to improve from, if not treated properly.

Bone out of skin!: But of course that would be obvious! usually severe swelling, inability to bear weight and point (focal) tenderness are suggestive signs. If there is deformity then that makes the diagnosis easier. An x-ray is always needed to make the definitive diagnosis.

Fracture: Pain, swelling, loss of motion or stiffness of the joint, deformity, difficulty bearing weight. This can be difficult to differentiate from a severe sprain or contusion without an xray.
