Difficult problem: Anterior knee pain is generally difficult to treat. Treatment usually is typical initially with nsaids, pt, bracing, occasional injections. Surgically, a scope to perform various tasks like chondroplasty, synovectomy, plica excision, and lateral retinacular release is done. Tibial tubercle osteotomy is moving up the chain of invasive ness. Patellectomy or patella-femoral replacement is rare.

Tough situation: Assuming mostly knee cap pain: (anterior knee). Other things that might help some: aggressive daily hamstring stretching, joint supplement (glucosamine-chondroiten). As for injections: was it a steroid? If you havent, may be worth trying a viscosupplementation injection (many on the market: synvisc, (hylan g-f 20) hyalgan, supartz, euflexxa).
