It could be ulcer: The complications of duodenal or gastric cancer are bleeding , perforation , penetration , obstruction of the pylorus etc. When there is penetration , the ulcer perforates posteriorly . This is felt as pain radiating to the back . I would recommend to see gastroenterologist or go to the ed depending on your condition. Blood works are very helpful but they are not always abnormal.
