Best thing. : Best thing to do is see a urologist. If it is that bad then of course go to the er.

Gabapentin: Gabapentin is an anti-seizure medication and is used for neuropthic pain. You're on quite high dose, if it is not effective for your pain, please discuss with your doctor for medication change. You might benefit from ganglion of impar block.
