Hard core Rx: This will respond well only to isotretinoin. Adjunct topical antibiotics & perhaps spironolactone may help as well. Nothing else will touch it and you'll repeat the course of rx over the years until it remits. It's not cheap, pleasant or risk-free but it is usually well worth it.
