Psychotherapy: You could try to see a therapist or psychologist. If there are things in your life that are making you depressed then you probably need to make life changes. If you are unable to identify why you feel depressed, seek a psychiatrist as well to discuss medication.

Medication and TMS: Sorry to hear of your travails. It is typical for people with severe depression. If you are this disabled, i would see a competent psychiatrist to evaluate your history and symptoms and get started on medication. If that does not work - either quickly enough or adequately enough for you to get on with your life and career - i would strongly consider tms (transcranial magnetic stimulation).
