Benfotiamine and: Alpha-lipoic acid based on brownlee's theory of complications and some medical journal data we use 600 mg of each , daily for patients in my practice.

Medical food: A prescription item, metanx, containing biologically active folate, b-12, and b-6 is indicated as an fda approved treatment for diabetic small fibre neuropathy, and it not only allows regrowth of nerve fibres but also can alleviate the burning. But, takes many months, perhaps up to 9-10. In meantime, Lyrica (pregabalin) or Cymbalta may help. There is no evidence that otc preps have any benefit.
