Is there anything that can cause low oxygen levels to not improve in a 6 year old?

Many things: Infection, poor air flow into the lungs, poor blood flow to and from the lungs, a heart problem such as a hole within the heart could all cause this. So could a lot of other things, all of which warrant thorough investigation.
