Usually no: Each dental procedure has a numeric code (for insurance purposes) describing that procedure. Root canal procedure codes are different than restoration codes. What was explained to you before treatment was rendered? You may have thought something was included when in fact it was not.

Usually no: You would have to ask your dentist that question, because a tooth that has a root canal usually needs a filling and then a crown. Most dentist, however, usually have a fee solely for the root canal, another for the filling and one for the crown, based on insurance codings. Each procedure in dentistry has a specific insurance code regardless is you don't have insurance.
