Could be: Many things from toenail fungus to something more serious. See a podiatrist to get a small sample for a pathology report to see exactly what it is. They will likely be able to tell from the looks of it too, but the report will tell for sure.

Possible hematoma: You may have an injury under the toenail which is causing the pain but has also created a blood blister (hematoma) under the nail plate. If the nail isn't already loose, you can see a podiatrist who could help drain the nail and/or avulse (remove) it safely as well as examined for an injury.
