I've been feeling more tired and have some urticaria. when should i see the doctor?

When you think so: Fatigue is a multifactorial concept and has many medical and non-medical causes, many which resolve by themselves (stress, sleep). Urticaria (hives) are considered acute if they occur for under 6 weeks. Acute hives are common and usually non-consequential.

If uncontrolled: Hives that are well controlled with antihistamines, such as zyrtec 10mg at bedtime with or without Allegra (fexofenadine) 180mg in the morning, never signify anything more sinister. However, if they persist despite dual antihistamines, you should see an allergist.
