Self harm: There are different types of self-harm, and gradations of intensity. Self-harm (self-cutting all the way to threatening suicide) can become a major coping style. Part of therapy involves developing healthier methods of dealing with pain and frustration. If you want to be proactive, you could ask your doctor about dbt -- it's a helpful treatment and skills program for this serious problem.

Self Harm: Be very direct with your doctor about your concerns and tell him or her that you don't feel that you are being taken seriously. If you still feel neglected consult with a new mental health professional in your area. Best wishes.
