Recognition and correction of the underlying process is the major step in the treatment of this serious condition. Intravenous administration of sodium bicarbonate (sb) has been the mainstay in the treatment of lactic acidosis. Dialysis, peritoneal in particular, can also be used in some cases to avoid a massive sb load.

Possible: There are different causes and types of rta. Vitamin d deficiency may cause it, different meds and toxins can do it, certain diseases (lupus, sjogren syndrome) may do it and finally there are some genetic causes. As you see, some of those are reversible and curable, while others (lupus, hereditary) are not, so RTA can only improve as long as the underlying cause is.

Resp or metabolic: Acidosis, means the blood's pH is lower than normal. Sometimes this is serious other times not. It is due to either a respiratory problem, not getting rid of carbon dioxide, or an imbalance in the bodies chemicals which can be from many problems.

Many causes: Kidney failure has to be ruled out first. Sepsis, i.e. Infection is important to rule out also. If has diabetes need to make sure it is not diabetic ketoacidosis. Also, important to make sure it is not due to metformin (glucophage) if takes it for diabetes. Other rare causes include Aspirin poisoning and antifreeze poisoning will be ruled out by his doctors.

See below: La arises from impairment of gluconeogenesis. Lactic acid is generated both in the liver and muscle and is oxidized to pyruvic acid and then converted via the gluconeogenenic pathway to g6p. Accumulation of g6p inhibits conversion of lactate to pyruvate. The Lactic Acid level rises during fasting as glucose falls. In people with gsd i, it may not fall entirely to normal even when gluc levels r nml.
