Be honest: Your doctor hasn't hear it all, but he has heard most of it. Be honest and explain your concerns. He/she will help you figure out whether this is a problem. And, if it is a problem he/she will help you determine a course of action to deal with it.

Just tell: Just go ahead and blurt it out. A good doctor should pay attention. If your doctor does not give you what you need, find a second opinion.
