Anatomy: Without an exam this is going to be a difficult question to answer. Your description leaves open numerous possibilities. It surely sounds a bit abnormal and thus my feeling that you need an examination in real life with your physician. But, in general boys in puberty have some growth of their nipples and surrounding tissue that will go away with time.
