In some cases: There are indications to leave an expander in for a longer time than usual. Especially if you are undergoing chemo or radio therapy treatments . Unless there are infections or soft tissue problems it should not cause any long term issues. This is something you need to discuss with your plastic surgeon, so as to plan your subsequent procedures.

It depends: Without specifics on your case the usual time for tissue expansion is 4-6 months. I try to complete my breast reconstructions within the year although not always possible. There is not added risk of leaving the te longer.

It is OK: Timing of breast tissue expansion in reconstruction varies. Decisions on timing are dependent on several factors including how you are feeling/healing and potential plans for postoperative chemotherapy and/or radiation. Sometimes expansion is delayed for months which can be quite normal. Although intended to be temporary devices, many are left in for many months without a problem.
