Checklists + exam: There are useful checklists on line. I would suggest googling your specific question - "how do I know if I have adhd?:" then..... Go see a doc or mental health care professional that can carefully review your history, your symptoms, your family history, and directly examine you. As with most things in medicine, diagnosis is based on good clinical history and a good exam.

Testing: By a clinical psychologist who specializes in assessment of childhood and adolescent disorders may help here. Treatment includes both medication by a psychiatrist as well as behavioral therapy by a psychologist who may specializize in this area. The research shows that the combined treatment is more effective than either treatment alone. Please also understand it's treatable and should not prevent you from achieving your goals in life.
