Nothing: If no symptoms like reflux then nothing. But if concerned see primary care doctor and get some tests like upper GI and endoscopy.

Depends: Not sure why you think you have a hiatal hernia, but generally, only large hernias cause problems such as difficulty eating, feeling full quickly, and chest pain/pressure. These are typically diagnosed with an upper GI contrast study (x-ray) that could be ordered by our primary care provider if necessary. Hope this helps!
