Get examined: Pain in the back of the heel towards the back of the leg is where achilles tendonitis occurs, but are you sure that's what you are dealing with? Get examined properly. Xrays can infer position of soft tissue but mainly highlight bone. To examine the achilles, an MRI would be warranted to check for damage - even a longitudinal tear which can be very subtle but result in lingering pain.

Achilles : Pain can be helped by, therapy, nsaids , stretching and continued use of orthotics. Topical compound creams can also help. Rest is the most important.

Physical therapy: Achilles tendinitis can be a difficult problem to treat. If you haven't received relief from the orthotics and pain medication, you may benefit from seeing a physician who can prescribe a course of focused ankle physical therapy in addition to making sure you are using the proper heel lift orthotics and prescribe appropriate pain medications. In very resistant cases, surgery is necessary.
