Start with MD: I would first speak to the physician who treats you for advice on the medication not working, and secondly how to deal with you getting proper dental treatment. If you don't feel that you are getting the proper advice, guidance and treatment, perhaps get a referral for another physician. You can get dental treatment with the proper sedation if necessary.

Slowly with patience: First, i would visit the dentists office without any intention of having work done. Visit and meet the dr. And his staff. Get acquainted with them and the office itself. Let the staff become familiar with you as well, and aware of your fears and anxiety. If u are comfortable with them, schedule a diff apt so you can work with them slowly to have the proper exam. For the cleaning try nitrous.
