Costs vary : By each case. You would need to be evaluated by your "perioprotect" dentist to determine treatment needs and cost. As far as insurance coverage is concerned, your dentist can do a pre- treatment estimate on your behalf. I always advise my patients that dental insurance is more of a contribution or donation towards the treatment if at all ! that's what keeps them all fortune 500 companies!
